How to Create an Email Questionnaire or Survey
Step 1: Generate HTML Email Questionnaire Form
Log in to your Spread account, navigate to Contacts > Get Login Form Code and click Click here for code on it.

Select target contact list from dropdown:
Click the Get Code button to get the below HTML codes shown:

Step 2: Add a add an additional line of codes at the end of several tags
Add an add Add the next line of code at the end of multiple tags. It allows submitting other fields besides email.
Email doesn’t support javascript so you need to delete the first line of code

These codes onsubmit=’return checkMail()’ must also be deleted.
Step 3: Add content of email questionnaire and Revise HTML
Spread supports the following extended fields:
first_name, middle_name, last_name, job_title, company_name, phone, Address1, address2, address3, city, state, country_id, post_code, fax , web_url , title, isMale, date1, date2
Custom1, Custom2, Custom3, Custom4, Custom5, Custom6, Custom7, Custom8, Custom9, Custom10, Custom11, Custom12, Custom13, Custom14, Custom15
Some general questions gen can be stored in the custom fields. For example: Question 1: Are you satisfied with our service? 1)Very satisfied 2)Somewhat satisfied 3)Satisfied 4)Dissatisfied 5)Very dissatisfied

If you have some knowledge of HTML, you can do Revise the HTML code to better suit your needs. After adding the above codes and making some HTML changes, the new HTML codes will appear as follows:
Satisfaction survey for fair distribution!
If you use the above HTML If you embed the code in your email, it will appear like this:
Satisfaction survey for fair distribution! Please enter your email address:
Step 4: Reply and receive email questionnaire reports
When your email subscribers submit your questionnaire/survey responses, you can find them in your Spread account under individual contacts.
Or you can export all questionnaire/survey responses to CSV file format.
